What it is

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every living cell, not a peptide, but tracked alongside research peptides because the same people researching cellular aging and longevity tend to be researching both. It's required for sirtuins, a family of enzymes involved in gene regulation and DNA repair, to function, and NAD+ levels are well documented to decline with age. Injectable NAD+ raises blood levels directly; the oral precursors nicotinamide mononucleotide (NMN) and nicotinamide riboside (NR) work differently, converting to NAD+ after absorption rather than delivering it directly.

What people use it for

Longevity and cellular energy are the dominant reasons, alongside cognitive clarity and general fatigue, which is where a lot of the anecdotal interest concentrates even though the trial data on cognition specifically is thin. The choice between injectable and an oral precursor usually comes down to how fast someone wants blood levels to move and how much the gut-conversion step of the oral forms matters to them.

Injectable NAD+, loading pattern

The pattern most commonly described, with no controlled trial behind it. An initial stretch of more frequent, smaller injections, commonly described as 50 to 100 mg two to three times a week, followed by a step down to a lower maintenance frequency once an initial effect is noticed. We couldn't find a source establishing this specific pattern; it's clinic and community convention, not trial-derived.

NMN, oral

Traces to a real published amount. A 2022 study in healthy older men used 250 mg per day for 12 weeks and found it significantly raised blood NAD+ levels, with a small improvement in grip strength and walking speed that the researchers themselves flagged as needing larger follow-up work to confirm.[1] A separate 2023 study used 125 mg twice daily, also 250 mg total, for 12 weeks.[2]

NR, oral

Also traces to real published amounts, though they vary more by study than NMN's do. A 2018 study in healthy middle-aged and older adults used 1,000 mg per day (500 mg twice daily) for 6 weeks and found oral NR well tolerated, reliably raising whole-blood NAD+ levels.[3] Other published work on NR has used lower daily amounts, commonly in the 250 to 300 mg range, so the published record doesn't converge on one single figure the way NMN's does.

Amount total, for planning your purchase: at an injectable maintenance pattern of roughly 100 mg twice a week, that's 200 mg a week, or a bit over 10 grams (10,400 mg) across a full year, which is around 10 to 20 of the common 500 mg or 1,000 mg vials depending on size. For NMN at its trial amount of 250 mg a day, that's roughly 7.5 grams a month and about 91 grams (91,250 mg) across a year. For NR at the higher end of its trial range, 1,000 mg a day, that's roughly 30 grams a month and about 365 grams across a year, a very different total from either the injectable form or the lower-amount NMN figure, since it's a daily oral dose rather than a periodic injection.

How much to reconstitute at once: injectable NAD+ is the one compound on this site where the general 28-day reconstituted-solution guideline does not apply, it's comparatively unstable in solution and the commonly described window is only 24 to 48 hours (see Storage and handling below). That makes a full 500 mg or 1,000 mg vial the wrong amount to reconstitute in one sitting for almost anyone, since a 100 mg dose means 80 to 90 percent of a 500 mg vial would need to be used within a day or two of adding water, not spread across weeks. Reconstituting only a small portion at a time, or working with a vendor that sells smaller vials sized closer to a single dosing session, avoids discarding most of what was paid for. Cycling guidelines

None of the three forms has a well-defined on-then-off cycling convention the way some shorter-course research peptides do. What's described instead, for all three, is sustained use over months, since the underlying premise, that NAD+ decline is a slow, ongoing process, doesn't lend itself to a short defined course. Some people describe cycling injectable NAD+ down to a lower maintenance frequency after an initial loading stretch, which is really a dose adjustment rather than a true off period.

Signs to stop or reconsider a given form:

  • Flushing, chest tightness, or a racing heartbeat during or shortly after an injectable NAD+ dose, which is reported more often at faster injection or infusion rates
  • Gastrointestinal discomfort with the oral precursors, which is the most commonly reported reason people reduce the oral amount or switch forms
  • No noticeable change after a reasonable, sustained trial, commonly described as several months, at which point re-evaluating the form or the goal makes more sense than continuing to increase the amount

Stacking

Expected results timeline

Early days: for injectable NAD+, some people report a subjective energy or mental clarity effect within days, though this is self-reported and not something the controlled trials measured directly. For NMN and NR, blood NAD+ levels rise measurably within the trial windows studied, generally a matter of weeks.

Weeks in: the Igarashi NMN trial measured its grip strength and gait speed findings at 12 weeks, and the Martens NR trial measured its blood NAD+ elevation over a similar window.[1][3] These are the actual timeframes the trial data covers.

Extended, continued use: the longevity and anti-aging effects that get the most public attention rest primarily on preclinical evidence from model organisms and mechanistic reasoning about sirtuin biology, not on long-run human outcome trials. Direct evidence that raising NAD+ levels extends human lifespan isn't available at any timeline yet.

What to expect realistically: a reliable rise in blood NAD+ is well established for the oral precursors specifically. Whether that biochemical change translates into a functional benefit you'd actually notice is the part that's still being worked out, and the published effect sizes on function have generally been modest.

Administration technique

  1. For injectable NAD+, the lyophilized powder is reconstituted with bacteriostatic water, at a concentration set by the reconstitution calculator for the target amount.
  2. The solution is swirled gently to dissolve rather than shaken, since agitation can degrade it, and NAD+ in solution is comparatively unstable, so mixing only what will be used within a day or two is common practice.
  3. The calculated volume is drawn into an insulin syringe or similar fine-gauge syringe.
  4. Injections are commonly given subcutaneously or intramuscularly, more slowly than most peptides, since faster injection is more often associated with flushing and chest tightness.
  5. For NMN or NR, the oral dose is simply taken with or without food, according to the product's own label, with no reconstitution involved.
  6. Logging the form, amount, and any subjective effect matters more here than with most compounds, since so much of what's tracked, energy and clarity in particular, is self-reported rather than measured.

Side effects and safety

Common: flushing, a racing or pounding heartbeat, and chest tightness during or shortly after an injectable NAD+ dose are the most consistently reported effects, and they're more common at faster injection or infusion speeds. Mild gastrointestinal discomfort is the most common complaint with the oral precursors.

Less common: injection-site irritation with the injectable form. Headache has been reported with both routes.

Stopping due to side effects: slowing the injection rate is the most commonly described fix for flushing and chest tightness before people stop the injectable form altogether. For the oral precursors, reducing the amount or splitting a single dose into two is the more common adjustment.

What we don't know: injectable NAD+ is less studied in controlled human trials than NMN or NR, so its safety picture rests more on clinic experience than on published trial data. Long-run outcomes from years of sustained use, in any of the three forms, aren't established.

Contraindications: no specific contraindications are well established for any of the three forms, which is itself a gap rather than a reassurance, since it usually reflects how little dedicated safety research exists rather than a demonstrated clean safety profile. Pregnancy, breastfeeding, and use in minors all have no research-grade safety data behind them and should be avoided.

Drug interactions: no interactions specific to NAD+ or its precursors are well characterized in the published literature. That's an absence of data, not evidence that none exist.

NAD+ vs. Epithalon

Injectable, lyophilized powder: refrigerate at 2 to 8°C; short-term room-temperature storage is commonly described as tolerable but not ideal. Protect from light.

Injectable, reconstituted solution: NAD+ is comparatively unstable once in solution. Use within 24 to 48 hours and avoid repeated freeze-thaw cycles; mixing only what will be used promptly is the more common approach than a large batch.

NMN and NR: stored at room temperature, sealed, away from heat and moisture, the same as most oral supplements.

Signs of degradation:
