FOXO4-DRI

FOXO4-DRI is a synthetic senolytic peptide designed to disrupt the FOXO4-p53 interaction that keeps senescent (aged, damaged) cells alive. Research spans senescent-cell clearance, tissue aging, and longevity, almost entirely in animal and cell models.

What it is

FOXO4-DRI is a synthetic peptide designed in a longevity research lab, where DRI stands for D-Retro-Inverso, a mirror-image construction that uses D-amino acids in reverse order to resist breakdown inside cells. It is built to mimic the region of the FOXO4 protein that binds the tumor-suppressor protein p53. By acting as a decoy, it interferes with the FOXO4-p53 interaction that senescent cells rely on to stay alive. [1] [2]

It belongs to a class of compounds called senolytics, agents studied for selectively clearing senescent cells (aged, damaged cells that stop dividing but linger and release inflammatory signals). FOXO4-DRI is not FDA-approved for any use and exists only as a research compound.

What researchers study it for
  • Senescent-cell clearance and healthspan The foundational study by Baar and colleagues designed FOXO4-DRI to disrupt the FOXO4-p53 interaction, and reported that it selectively triggered death of senescent cells while restoring fitness, fur density, and kidney function in aged and fast-aging mice. [1]
  • The FOXO4-p53 binding mechanism Structural work using NMR spectroscopy identified the disordered p53 transactivation domain as the binding target of both FOXO4 and FOXO4-DRI, helping explain how the peptide displaces p53 inside senescent cells. [2]
  • Age-related hormonal decline In naturally aged mice, researchers studied whether clearing senescent Leydig cells with FOXO4-DRI would improve the testicular microenvironment, and reported reduced age-related testosterone insufficiency. [3]
  • Tissue and cartilage applications In lab studies of human chondrocytes (cartilage cells) expanded for repair procedures, FOXO4-DRI was studied for selectively removing senescent cells that accumulate during expansion and degrade the quality of the cell material. [4]
  • Brain aging and cognition A 2026 review surveyed preclinical evidence for targeting the FOXO4-p53 axis with retro-inverso peptides as a strategy studied for reducing brain aging and cognitive decline, while noting the absence of human data. [6]
  • Context-dependent risks of senescent-cell clearance Not all senescent-cell removal is beneficial. In rodent models, eliminating senescent cells (including with FOXO4-DRI) was associated with worse pulmonary hypertension, a finding researchers flag as a caution against assuming senolytics are uniformly protective. [5]
Research context

FOXO4-DRI's evidence base is almost entirely preclinical, built from cell studies and animal models rather than human trials. The landmark 2017 Cell paper established the compound and the FOXO4-p53 mechanism, and most subsequent work extends that foundation into specific tissues: cartilage cells, Leydig cells, fibroblasts, and the brain. [1] [3] [4] There are no published human dosing studies and no established human safety data.

The honest counterweight in this literature is the pulmonary hypertension work, which found that clearing senescent cells can make a serious condition worse in some contexts. [5] Senescent cells are not simply "bad cells" to be removed; in certain tissues they serve protective roles, and the timing and location of clearance appear to matter. Researchers reviewing FOXO4-DRI should treat the senolytic story as biologically interesting but unsettled, and should distinguish the controlled laboratory peptide from the research-grade material sold by vendors, which is not equivalent.
